New Patient Information

  • We will be contacting your referring physician for a copy of your medical records, via fax, for the past year (including most recent labs, radiology reports, discharge summaries, office notes, etc).
  • Please remember to complete the forms (located under Form Download) prior to your appointment.
  • Bring a list of all medications and their dosages.
  • Insurance cards are required at each visit.
  • Copayments are payable at the time of the visit. We accept cash, checks, and VISA/MasterCard & Discover.
  • If your insurance requires a referral (HMO), you must have the referral sent to our office no later than the day before your appointment. Otherwise you cannot be seen the day of your scheduled appointment. It is your responsibility to obtain and request the referral for your visit.
  • You will need to give a urine specimen upon arrival.
  • Be informed that you will be here for approximately one hour for your visit.
  • Please bring a photo ID to your visit.
  • We require at least 24 hours cancellation notice.  If appointments are not cancelled with 24 hours notice, you will be charged a $50.00 fee.  To cancel your appointment, please call 610-649-1175
